What is a DOT Physical?

A Department of Transportation (DOT) physical is a health examination mandated by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle drivers.

The goal of the DOT physical exam is to determine whether a driver is physically and mentally capable of safely operating a commercial vehicle. This comprehensive evaluation helps ensure the safety of both the driver and the public on our roads.

During the examination, a certified medical examiner will evaluate your overall health, vision, hearing, blood pressure, and other factors that could impact your ability to drive safely for extended periods.

Who Needs a DOT Physical?

  • Commercial driver's license (CDL) holders
  • Drivers operating vehicles with gross vehicle weight rating (GVWR) of 26,001+ lbs
  • Drivers transporting 16+ passengers (including the driver)
  • Drivers transporting hazardous materials requiring placards

What to Expect During Your Exam

Health History Review

Complete a detailed questionnaire about your medical history, medications, and any current health conditions.

Vital Signs Check

Blood pressure, pulse, and temperature will be measured to assess overall cardiovascular health.

Physical Examination

A thorough physical exam including vision, hearing, lung function, and neurological assessments.

Urinalysis

Standard urine test to check for underlying health conditions (not a drug test, but may include one).

Certification

Receive your Medical Examiner's Certificate (MEC) and get entered into the FMCSA National Registry.

Requirements

DOT Physical Requirements

To pass your DOT physical, you must meet specific medical standards set by the FMCSA.

Vision

  • 20/40 vision in each eye
  • 70° peripheral vision (both eyes)
  • Able to recognize colors

Hearing

  • Hear whispered voice at 5 feet
  • Or average hearing loss ≤ 40 dB

Blood Pressure

  • Below 140/90 is ideal
  • May need medication

Physical Fitness

  • Able to perform all vehicle tasks
  • Normal range of motion
  • No impairments affecting driving

Medical Conditions

  • No insulin-dependent diabetes
  • No seizure disorders
  • No substance abuse issues

Certification Period

  • Standard: 2 years
  • 1 year if monitored condition
  • 3 months if borderline BP
